We offer flexible scheduling and multiple clinic location options based on provider availability and business needs. *Please note: All candidates, including those seeking part-time opportunities, must be available to complete the required training program.* **Practice Locations & Schedule Options** **Full-Time Opportunities** **Option 1: Longmont & Thornton** - Primarily based in Longmont and Thornton - Occasional coverage in Fort Collins (typically no more than twice monthly) **Option 2: All Three Clinics** - Approximately 2 days per week in Longmont - 2 days per week in Thornton - 1 day per week in Fort Collins **Part-Time Opportunities** **Option 1: Fort Collins** - 1 to 2 days per week **Option 2: Longmont & Thornton** - 3 to 4 days per week across both clinics Specific clinic assignments and workdays will be determined in collaboration with the hiring team. Candidates are encouraged to share their location preferences and weekly availability. **Clinic Hours** - Monday, Thursday, Friday: 8:00 AM - 4:00 PM - Tuesday, Wednesday: 9:00 AM - 5:00 PM **Position Overview** This is a **100% outpatient, office-based, non-procedural role** focused on consultation, evaluation, and ongoing management of patients with vascular disease. Providers will care for patients with conditions including: - Venous insufficiency - Peripheral arterial disease (PAD) - Pelvic congestion syndrome - Other vascular disorders The APP will work closely with our vascular physicians while practicing independently within their scope once fully trained.
