## About the role

- Role: Physical Therapist (PT)

- Schedule: Full-time Monday–Friday + occasional weekends (as needed)

- Location: Field Based

- Compensation (Productivity Points): $75.00 / point | $100,000-$130,000/year

We’re building our team in **Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care.**

#### The mission of this role

**Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.**

#### Objectives

1. Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality.

2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

#### What you'll own

- Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

- Complete comprehensive in-home assessments and deliver skilled PT visits.

- Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

- Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.

- Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

- Manage treatment and outcomes directly, including PTA supervision where appropriate.

- Coach patients and caregivers with practical home programs that actually get done.

- Document promptly and thoroughly — defensible, efficient, and consistent.

#### Who you are / how you operate

- Dedicated clinician: home health experience strongly preferred

#### Must-haves

- Active Michigan PT license (in good standing)

- Strong clinical judgment and comfort working independently in the field

- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly

- Strong communication and documentation

- Reliable follow-through on orders, frequencies, and patient goals

- Current driver’s license and comfort with daily travel across assigned territory

- Able to carry/transport basic therapy equipment as needed

- Competency in core home health PT scopeSafety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

- Strengthening, functional mobility, and home safety assessment/modification education

- Patient/caregiver coaching with practical home programs

#### Nice-to-haves

- Prior home health experience

- Deeper clinical experience in high-signal PT populations

- Experience supervising/mentoring PTAs and collaborating on care plans

- Strong organization and time management
