## About the role

**Physical Therapist (PT) - Home Health**

- Schedule: Full-time Monday–Friday + occasional weekends (as needed)

- Location: Field Based

- Compensation (Productivity Points): $100,000-130,000/year $75.00/point

##### Who We Are:

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to **dramatically improve patient access to home health care**.

The PT role is a core part of that mission: you restore function, keep patients safe at home, and drive real outcomes.

We also build technology to support field clinicians. Our **charting product** reduces documentation burden, and our **scheduling product** (launching soon) will make routing and visit coordination easier — protecting your time.

##### What you will do:

- Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

- Complete comprehensive in-home assessments and deliver skilled PT visits.

- Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

- Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.

- Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

- Manage treatment and outcomes directly, including PTA supervision where appropriate.

- Coach patients and caregivers with practical home programs that actually get done.

- Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

##### Requirements

**Must-haves**

- Active Texas PT license (in good standing)

- Strong clinical judgment and comfort working independently in the field

- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly

- Strong communication and documentation

- Reliable follow-through on orders, frequencies, and patient goals

- Current driver’s license and comfort with daily travel across assigned territory

- Able to carry/transport basic therapy equipment as needed

- Competency in core home health PT scopeSafety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

- Strengthening, functional mobility, and home safety assessment/modification education

- Patient/caregiver coaching with practical home programs**Nice-to-haves**

- Prior home health experience

- Homecare Homebase (HCHB) experience (or similar home health EMR)

- Deeper clinical experience in high-signal PT populations

- Experience supervising/mentoring PTAs and collaborating on care plans

- Strong organization and time management
