## About the role

- Schedule: Full-time Monday–Friday + occasional weekends (as needed)

- Location: Field-based

- Comp: $75.00 / point | $100,000 - $130,000 /year

##### Who We Are

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our ultimate mission is to **dramatically improve patient access to home health care.**

The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our **charting product** is designed to reduce documentation burden and keep visits moving, and our **scheduling product** (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.

If you have strong clinical judgment and you like owning outcomes, this role gives you autonomy, meaningful patient impact, and a support team built to remove administrative friction.

##### What you will do

- Complete in-home OT evaluations and deliver skilled OT visits.

- Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.

- Recommend and train on adaptive equipment and home modifications as needed.

- Coordinate with nursing and the broader care team when condition or needs change.

- Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

##### Requirements

**Must-haves**

- Active Texas OT license (in good standing)

- Strong clinical judgment and comfort working independently in the field

- Comfort escalating changes in condition to RN/MD and documenting rationale clearly

- Strong communication and documentation

- Reliable follow-through on orders, frequencies, and patient goals

- Current driver’s license and comfort with daily travel across assigned territory

- Able to carry/transport basic therapy equipment as needed

- Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)**Nice-to-haves**

- Prior home health experience

- Homecare Homebase (HCHB) experience (or similar home health EMR)Role: Occupational Therapist (OT)
